Here is the coaching contract I use. Please fill out the form at the bottom to accept this contract. If you have any questions, please feel free to contact me before submitting the form.

This document is a contract between you and your coach, Sandy Walker. The services to be provided are coaching sessions with you, the client. When you begin coaching under this contract, you are responsible to make payment for your sessions. Fees for coaching sessions will be paid at time of service by credit card or PayPal. Except for emergencies, please give 12-24 hours notice if you need to cancel or change the time of an appointment. Professional time spent outside of coaching sessions will be billed on a prorated basis rounded to the nearest quarter of an hour. You agree to pay for any additional services within 30 days. Current hourly rates are listed on the website.

Coaching versus Psychotherapy

While there are some similarities between coaching and psychotherapy, they are very different activities and it is important that you understand the differences between them. By agreeing to this contract, you state that you understand that you are not receiving psychotherapy from your coach, regardless of their training. If, during the course of this contract, it becomes clear that you would benefit from receiving psychotherapy, you will be notified and referred to a therapist. You may or may not be able to continue receiving coaching sessions – this will be determined by both you and your coach when/if the need arises.

The focus of coaching is the development and implementation of strategies to reach your goals. Coaching may address specific personal projects, life balance, job performance and satisfaction, or general conditions in your life, business, or profession.

The relationship between the coach and client is specifically designed to avoid the power differentials that can occur in the psychotherapy relationship. You, the client, set the agenda and the success of the enterprise depends on your willingness to take risks and try new approaches. The relationship is designed to be direct and challenging. You can count on your coach to be honest and straightforward, asking pointed questions and using challenging techniques to move you forward. You are expected to evaluate your progress and if coaching is not working as you wish, you should immediately inform your coach so you can both take steps to correct the problem.

You agree by signing this contract that you will not hold Sandy Walker liable for any consequences of the decisions you make as a result of your coaching sessions. Because you will be making changes in your life, these changes may create awkward or uncomfortable experiences in your home, at your job, among your family, friends or neighbors. Your coach is there to help you decide what you want to do, but ultimately, you take full responsibility for the decisions you make and the consequences of those decisions. Your coach will not go to court on your behalf for any reason. You also agree not to take any legal action against Sandy Walker for any advice you receive as a coaching client.

It is important to understand that coaching is a professional relationship. While it may often feel like a close personal relationship, it is not one that can extend beyond professional boundaries both during and after the contract is completed. In order to maintain professional boundaries, your coach will not attend family parties, weddings, baptisms, graduations, etc. Personal gifts are also not expected – either for or from your coach. Tokens of appreciation can be extended as long as they do not have a high monetary value.

Confidentiality

Sandy Walker agrees to protect the confidentiality of your communication. No information about you will be released to others without your written permission. There are some situations in which your coach may be legally obligated to breach confidentiality in order to protect you or others from harm. If you disclose information that indicates that a child, elderly or disabled person is currently being abused or is in danger of abuse, your coach will report that to the appropriate state agency. If you disclose that you intend to kill or harm yourself, or if you make threats of imminent violence against another, your coach will take protective actions including notifying authorities who can protect you. These situations are very rare in coaching sessions but you should be aware of the limits of confidentiality in this contract.

By agreeing to this contract, you agree that you are aware that it is impossible to protect the confidentiality of information that is transmitted electronically. This is particularly true of e-mail and information stored on computers that are connected to the internet. This is also true of cell phone communications.

For Parent/Legal Guardian:
I understand that for this coaching agreement to have the most benefit for my child, I will not be told the details of what is discussed between the coach and my child. I can be informed of the general experience my child is receiving from coaching upon request. I have read the coaching contract and I agree to the terms of the contract for my child. I give my full consent for my child to be coached in the manner described in the contract.
